Stay in your own home, on your own terms

Hands-on daily-living support for adults who want to keep living in their own home and community rather than move somewhere more restrictive.

Overview

What this service is

Individual community living support (ICLS) is for adults who can live on their own with the right help at the right moments. Staff visit on a regular schedule and work through the person’s day with them: medications, meals, appointments, bills, the bus route, the pharmacy.

The service is deliberately flexible. Some people need a daily check-in; others need a few focused hours a week. We build the schedule around the person’s life, and adjust it as their needs change.

Common questions

Things families and case managers ask us

How is ICLS different from individualized home supports?

ICLS focuses on hands-on support to get through daily life and stay safely at home. Individualized home supports with training focuses on teaching and practicing skills toward greater independence. Many people benefit from both, and we deliver both.

Does the person need to live alone?

No. ICLS can be provided to someone living alone, with roommates, or with family, as long as the support is focused on the individual.

Can the schedule change?

Yes. Plans are reviewed regularly, and hours can be increased or reduced with the case manager as the person’s needs shift.
