Eight services,
built around one person at a time

Every service is delivered under our Minnesota 245D license, with a support plan shaped around each person’s routines, goals, and the people who matter most to them.

How services fit together

Most people need more than one

A person might need a homemaker on Tuesdays, a companion on Thursdays, night supervision every night, and a few hours of skills training in between. Rather than juggling providers, families and case managers work with one Goldenview team that knows the whole picture.

  • One intake, one support plan, one point of contact
  • Staff who share notes across services, so nothing falls through the cracks
  • Hours that can shift between services as needs change
  • Direct coordination with county and tribal case managers
Eligibility & funding

How services get authorized

Our services are licensed under Minnesota Statute 245D and are most often funded through Minnesota Medicaid waiver programs. Here’s the short version.

01

Assessment

A county or tribal case manager completes a MnCHOICES assessment to determine eligibility and which supports are needed.

02

Support plan

Services are written into the person’s support plan, with authorized hours for each. We help you and the case manager choose the right ones.

03

Service agreement

Once authorized, we finalize a service agreement, match staff, and set a start date, usually within days.

Ready when you are

Let’s talk about the right fit

Call, email, or send a referral. A person answers within one business day and helps you figure out which services fit and how to get them authorized.
