Three steps from first call to first shift
Whether you’re a family member, a county or tribal case manager, or reaching out for yourself, the path is the same. It all starts with one conversation.
Simple on purpose
No forms to hunt down and no phone tree. Tell us a little about the person, and we take it from there.
Reach out
Call (612) 558-3006, fax (612) 329-0013, email contact@goldenviewmn.com, or use the form below. Tell us a little about who needs support, and we’ll get back to you within one business day.
Plan together
We meet the person and the people who know them best, review the service agreement, and write a support plan around their goals and routines.
Support begins
Matched staff start on a schedule that fits the household. As life changes, we keep checking that the plan still fits.
Anyone who knows the person
You don’t need to have everything figured out before you call. Here’s what’s helpful to have ready, depending on who you are.
Families & caregivers
Tell us about your person and what’s hard right now. If they already have a case manager, share their name; if not, we’ll help you find the right county contact.
Case managers
Send the support plan, assessment summary, and the services and units you’re authorizing. We’ll confirm capacity and return a service agreement quickly.
Self-referrals
Reaching out for yourself? Great. Call or write and tell us what kind of support would make life easier. We’ll explain the options and next steps in plain language.
Send us the basics
Share what you can. We’ll follow up by phone or email within one business day to fill in the rest. Please don’t include medical records or other sensitive documents here; we’ll arrange a secure way to exchange those.
Before you reach out
Still have questions? Contact us and ask. There’s no wrong first question.
Do I need a case manager before I contact you?
No. Many families reach out first. If the person doesn’t yet have a case manager or an assessment, we’ll explain how to request one through the county and help you get started.
How are services paid for?
Most of our services are funded through Minnesota Medicaid waiver programs after a county or tribal assessment. Private pay is also an option. We’ll be upfront about costs and funding before anything starts.
How soon can services start?
Once services are authorized and a service agreement is in place, we typically match staff and begin within days. Urgent situations are prioritized; tell us if timing is critical.
What areas do you serve?
We’re based in Minneapolis and serve families across the Twin Cities metro. If you’re unsure whether we cover your area, ask; we’ll tell you plainly.
Can we meet you before deciding?
Yes, and we encourage it. A no-obligation conversation, at your home or our office, is the best way to see whether we’re the right fit.
